A search log helps you keep track of calls and show where the search is getting stuck. Record what happened as well as how many providers you contacted.
Set up the columns once
Use provider name, contact date, contact method, service requested, response, and next step. Add separate columns for possible start date and reasons the arrangement would not work. Examples include no opening, location too far away, or inability to accept the authorization. Record the provider's actual explanation.
Separate unanswered calls from unavailable providers
A voicemail isn't a confirmed refusal. Mark it as awaiting response and record any later follow-up. When a provider suggests someone else, enter that referral on a new row. Avoid listing the same agency under several names and accidentally overstating the number of distinct providers contacted.
Use the log to ask for help
Summarize the barriers in a short message to the relevant school contact or representative. For example, explain that the available openings conflict with the authorized schedule or that no contacted provider accepts the paperwork. Keep the full log private. The search record may help explain the delay, but it doesn't by itself establish a particular remedy.
