IEP Service Logs Do Not Match: What Parents Can Do

If a service log conflicts with your calendar or your child's attendance, identify the exact entries you question. Specific questions are easier to answer than a claim that all the records are wrong.

Preserve the original record

Keep the file exactly as you received it and make your notes on a separate copy. Record when you received it and the period it covers. Check abbreviations, dates, units, and provider names before deciding that an entry is incorrect. Ask for explanations of unfamiliar codes.

Create a short discrepancy list

Use columns for the service date, what the school record says, what your information says, and the document supporting your concern. A message canceling an appointment or an attendance entry may help clarify a particular date. Mark uncertain information as uncertain, and avoid filling missing dates with assumptions.

Request clarification in writing

Send the specific entries and ask the school to explain or review them. Keep its response alongside your comparison. If the explanation doesn't resolve it, ask about the record-review process and discuss the discrepancy with your representative. Don't alter a service log or sign an attendance statement that you believe is inaccurate merely to move paperwork along.

Questions families ask

Does a blank record prove a session never happened?

Not by itself. It identifies a question to investigate using the school's explanation and other available records.
